Tool grinding: Form cutter sharpening attachment

I recently read the Tafe chapter on tool and cutter grinding. The drawings in it are from a Jones Shipman universal grinder, which looks a lot like my AI Hembrug U2 and that's a copy of a Cincinnati.
In this text there is a reference to a "form cutter sharpening attachment", with a drawing of it:

In one of the boxes with "tooling to be identified" belonging to my grinder there is most of what I now recognized to be one of those attachments.

As far as I know AI Hembrug never made such a thing, so most likely it is a Cincinnati part. (there are more parts which I think are Cincinnati in those boxes).
Of course the key words are "most of", and since my grinder is a good copy, the Cincinnati attachment will fit my grinder and I would like to complete it.
Does anyone have this attachment and is willing to take some pictures and possibly measure the key dimensions? This would make it possible for me to fabricate the missing parts.
