Machining a D hole in a gear/pulley

Here's a question for the brainiacs on here. How is a "D" hole machined in a gear or pulley? Looked up on google, but all it shows is cutting a keyway. Have a copy of Machine Design Theory and Practice nothing in there either, that I could find.
