A $200+ bandsaw mill blade driven by a 27HP motor comes off at full speed (5000 fpm)
90% of the teeth were either missing, broken or bent.
This is 1m of the 6m long blade - the photos don't really capture the damage.
The gullet damage showed how close the blade came to breaking

The blade half wrapped itself around the drive wheel and there was a very loud screeching and lots of sparks.
On the trailing wheel side the blade snaked out from under the guard and flayed around for a couple of seconds until the blade got jammed up against the drive wheel axle.
Fortunately much of the exposed blade was deep inside a cut and I was standing well out of the way - well about a 1/2m away.
I could not remove the damaged blade without dismantling the mill and after taking off a few parts I realized the blade was toast and so I cut it off with an angle grinder.
Blades have come off a couple of times at idle when backing out of a cut whereby the blade just flops onto the log/ground with very little blade damage that cannot be fixed by a resharpen.
Boss was quite non-plussed after he found out I was OK.
The cause is likely to be a combination of factors mainly trying to push a blunt blade too fast through a very dirty log.
